Each year, the Pennsylvania Training & Technical Assistance Network (PaTTAN) creates a Secondary Transition Resource for students who will turn 14 during the curent school year. At age 14, secondary transition planning begins for all students with disabilities. The Secondary Transition Resource helps students with disabilities and their families get information about what to expect as the student transitions from high school and what sorts of agencies and resources can help.
A Secondary Transition
is to provide youth, young adults, parents, and professionals with secondary transition
resources to support a young persons progress toward the attainment of post-secondary
goals related to education, employment, and community living.
